MCAT EXPERTISE

You may be surprised at the low percentage of questions associated with this chapter, given that this chapter touches on several classic MCAT topics including pressure, gases, and equilibrium. And you may see questions on your MCAT that relate to the work done on air in the lungs, for example. However, such a question would depend much more on your general knowledge of work, energy, and gases (regardless of context) than your knowledge of the respiratory system specifically.

Introduction

Coughing. Fever. Shortness of breath. Hypoxia. All are symptoms of a number of pulmonary diseases, from a flareup of chronic obstructive pulmonary disease (COPD), to Streptococcus pneumoniae (pneumococcal) pneumonia, to a type of hypersensitivity pneumonitis known as extrinsic allergic alveolitis (EAA). This last example is a bit more esoteric and can be brought on by hypersensitivity to anything from dried grass, to rat urine, to mold that grows in hot tubs—what is sometimes called hot tub lung. Not all cases of hot tub lung are severe, but certainly none are enjoyable. They are often misdiagnosed as asthma or bronchitis and may be treated with steroids, which quell the immune system and reduce inflammation. Because hot tub lung can potentially go away by itself, antibiotic therapy is not always recommended. As a physician, you may end up simply having to tell your patients that the best way to avoid hot tub lung is to make sure that the tub is cleaned properly and routinely before use.

The lesson here isn’t to avoid hot tubs. It’s that the lungs are essential, sensitive organs with delicate membranes that must be protected. Many types of stressors (pathogens, particles, or chemicals) can irritate them and cause respiratory distress. In this chapter, we’ll look at the structure of the lungs and the microanatomy of respiration. We’ll also talk about the mechanics of breathing, as well as the overall function of the lungs.

6.1 Anatomy and Mechanism of Breathing

The lungs are located in the thoracic cavity, the structure of which is specially designed to perform breathing.

Anatomy

The anatomy of the respiratory system is summarized in Figure 6.1. Gas exchange occurs in the lungs. Air enters the respiratory tract through the external nares of the nose and then passes through the nasal cavity, where it is filtered by mucous membranes and nasal hairs (vibrissae).

KEY CONCEPT

The nose and mouth serve several important roles in breathing by removing dirt and particulate matter from the air and warming and humidifying it before it reaches the lungs.

Next, air passes into the pharynx and the larynx. The pharynx resides behind the nasal cavity and at the back of the mouth; it is a common pathway for both air destined for the lungs and food destined for the esophagus. In contrast, the larynx lies below the pharynx and is only a pathway for air. To keep food out of the respiratory tract, the opening of the larynx (glottis) is covered by the epiglottis during swallowing. The larynx contains two vocal cords that are maneuvered using skeletal muscle and cartilage. From the larynx, air passes into the cartilaginous trachea and then into one of the two mainstem bronchi. The bronchi and trachea contain ciliated pseudostratified columnar epithelial cells to catch and remove particulate that have made it past the mucous membranes in the nose and mouth.

In the lungs, the bronchi continue to divide into smaller structures known as bronchioles, which divide further until they end in the tiny balloon-like structures in which gas exchange occurs (alveoli). Each alveolus is coated with surfactant, a detergent that lowers surface tension and prevents the alveolus from collapsing on itself. A network of capillaries surrounds each alveolus to carry oxygen and carbon dioxide. The branching and minute size of the alveoli allow for an exceptionally large surface area for gas exchange—approximately 100 m2  in total.

REAL WORLD

The left lung has a small indentation, making it slightly smaller than the right lung. It also contains only two lobes, while the right lung contains three. Why might this be? It is due to the position of the heart in the thoracic cavity.

The lungs themselves are contained in the thoracic cavity, which also contains the heart. The chest wall forms the outside of the thoracic cavity. Membranes known as pleurae surround each lung, as shown in Figure 6.2. The pleura forms a closed sac against which the lung expands. The surface adjacent to the lung is the visceral pleura, and the outer part is the parietal pleura.

The lungs do not fill passively, and require skeletal muscle to generate the negative pressure for expansion. The most important of these muscles is the diaphragm, a thin, muscular structure that divides the thoracic (chest) cavity from the abdominal cavity. The diaphragm is under somatic control, even though breathing itself is under autonomic control. In addition, muscles of the chest wall, back, and neck may also participate in breathing, especially when breathing is labored due to a pathologic condition.

Breathing

Before we discuss breathing itself, it is worth taking a closer look at the relationship between the pleurae and the lungs. Imagine that you have a large, partially deflated balloon. Now, imagine taking your fist and pushing it against the balloon so that the balloon comes up and surrounds your hand. This is analogous to a lung and its pleura. Our fist is the lung, and the balloon represents both pleural layers. The side directly touching our fist is the visceral pleura, and the outer layer is the parietal pleura, which is associated with the chest wall in real life. The space within the sac is referred to as the intrapleural space, which contains a thin layer of fluid. This pleural fluid helps lubricate the two pleural surfaces. The pressure differentials that can be created across the pleura ultimately drive breathing, as we explore in the next section.

REAL WORLD

The intrapleural space is an example of a potential space—a space that is normally empty or collapsed. In some pathologic states, that potential space can be expanded by fluid or air that accumulates between the two pleural layers. For example, in a pleural effusion, fluid accumulates in the intrapleural space. In a pneumothorax, air collects here. Each of these states disturbs the normal mechanics of the breathing apparatus and can cause atelectasis, or lung collapse.

Let’s turn to the mechanics of ventilation, which are grounded in physics. As discussed in Chapters 2 and 3 of MCAT Physics and Math Review, we can use pressure to do useful work in a system. Here, we use pressure differentials between the intrapleural space and the lungs to drive air into the lungs.

Inhalation

Inhalation is an active process. We use our diaphragm as well as the external intercostal muscles (one of the layers of muscles between the ribs) to expand the thoracic cavity, as shown in Figure 6.3. As the diaphragm flattens and the chest wall expands outward, the intrathoracic volume (the volume of the chest cavity) increases. Specifically, because the intrapleural space most closely abuts the chest wall, its volume increases first. Can we predict what will happen to intrapleural pressure? From our understanding of Boyle’s law, an increase in intrapleural volume leads to a decrease in intrapleural pressure.

BRIDGE

Boyle’s law states that, at a constant temperature, the pressure and volume of gases are inversely related. This is the principle underlying negative-pressure breathing: as the chest wall expands, the pressure in the lungs drops, and air is drawn into the lungs.

Now we have low pressure in the intrapleural space. What about inside the lungs? The gas in the lungs is initially at atmospheric pressure, which is now higher than the pressure in the intrapleural space. The lungs will therefore expand into the intrapleural space, and the pressure in the lungs will drop. Air will then be sucked in from a higher-pressure environment—the outside world. This mechanism is referred to as negative-pressure breathing because the driving force is the lower (relatively negative) pressure in the intrapleural space compared with the lungs.

Exhalation

Unlike inhalation, exhalation does not have to be an active process. Simple relaxation of the external intercostal muscles will reverse the processes we discussed in the last paragraph. As the diaphragm and external intercostals relax, the chest cavity decreases in volume. What will happen to pressure in the intrapleural space? It will go up, again explained by Boyle’s law. Now pressure in the intrapleural space is higher than in the lungs, which is still at atmospheric pressure. Thus, air will be pushed out, resulting in exhalation. During active tasks, we can speed this process up by using the internal intercostal muscles and abdominal muscles, which oppose the external intercostals and pull the rib cage down. This actively decreases the volume of the thoracic cavity. Finally, recall that surfactant prevents the complete collapse of the alveoli during exhalation by reducing surface tension at the alveolar surface.

KEY CONCEPT

Inhalation and exhalation require different amounts of energy expenditure. Muscle contraction is required to create the negative pressure in the thoracic cavity that forces air into the lungs during inspiration. Expiration during calm states is entirely due to elastic recoil of the lungs and the musculature. During more active states, the muscles can be used to force air out and speed up the process of ventilation.

Remember the balloon analogy from before. The lungs have a resilient, elastic quality and are attached via the pleurae to the chest wall. The chest wall expands on inhalation, pulling the lungs with it and creating the pressure differential required for inhalation. As the chest wall relaxes, the lungs recoil due to the intrinsic elastic quality of the lungs and surface tension in the alveoli, accentuating the relaxation process. When the lungs recoil, their volume becomes smaller, and the pressure increases. Now the pressure inside the lungs is higher than the outside pressure, and exhalation occurs. Note that the indirect connection of the lungs to the chest wall also prevents them from collapsing completely on recoil, like surfactant.

REAL WORLD

Emphysema is a disease characterized by the destruction of the alveolar walls. This results in reduced elastic recoil of the lungs, making the process of exhalation extremely difficult. Most cases of emphysema are caused by cigarette smoking.

Lung Capacities and Volumes

In pulmonology (the medical field associated with the lungs and breathing), we frequently must assess lung capacities and volumes. One instrument used to measure these quantities is a spirometer. While a spirometer cannot measure the amount of air remaining in the lung after complete exhalation (residual volume), it provides a number of measures that are useful in clinical medicine.

Regulation of Breathing

Breathing requires input from our nervous control center. Ventilation is primarily regulated by a collection of neurons in the medulla oblongata called the ventilation center that fire rhythmically to cause regular contraction of respiratory muscles. These neurons contain chemoreceptors that are primarily sensitive to carbon dioxide concentration. As the partial pressure of carbon dioxide in the blood rises (hypercarbia or hypercapnia), the respiratory rate will increase so that more carbon dioxide is exhaled, causing carbon dioxide levels in the blood to fall. These cells also respond to changes in oxygen concentration, although this tends to have significance only during periods of significant hypoxemia (low oxygen concentration in the blood).

We can, to a limited extent, control our breathing consciously. We can choose to breathe more rapidly or slowly; however, extended periods of hypoventilation would lead to increased carbon dioxide levels and an override by the medulla oblongata (which would jump-start breathing). The opposite process (hyperventilation) would blow off too much carbon dioxide and ultimately inhibit ventilation.

6.2 Functions of the Respiratory System

After Chapter 6.2, you will be able to:

No organ system functions alone. The lungs function in gas exchange, but this is only part of the respiratory story. The lungs are lined with a tremendous number of capillaries that can also be used in thermoregulation. The lungs also represent a pathway into the body and serve an immune function to prevent invaders from gaining access to the bloodstream. Finally, the lungs also allow for control of blood pH by controlling carbon dioxide concentrations. Therefore, the lungs are integrated with many other body systems, including the cardiovascular, immune, renal, and nervous systems.

Gas Exchange

Gas exchange is, of course, the primary function of the lungs. Each alveolus is surrounded by a network of capillaries. The capillaries bring deoxygenated blood from the pulmonary arteries, which originate from the right ventricle of the heart. The walls of the alveoli are only one cell thick, which facilitates the diffusion of carbon dioxide from the blood into the lungs, and oxygen into the blood. The oxygenated blood returns to the left atrium of the heart via the pulmonary veins.

REAL WORLD

Diffusion of gases occurs across a very thin membrane between the alveolus and the capillary. However, certain diseases may cause fibrosis, or scarring, of this membrane, resulting in less effective diffusion. Other diseases may cause a limitation of ventilation (gas flow) or perfusion (blood flow) to the lung. All of these mechanisms can cause hypoxia—low blood oxygen levels—although they accomplish that same end result through different means.

The driving force for gas exchange is the pressure differential of the gases. When it initially arrives at the alveoli, blood has a relatively low partial pressure of oxygen and a relatively high partial pressure of carbon dioxide, facilitating transfer of each down its respective concentration gradient, as shown in Figure 6.5. Because the gradient between the blood and air in the lungs is already present as the blood enters the lungs, no energy is required for gas transfer.

KEY CONCEPT

O2 in the alveoli flows down its partial pressure gradient from the alveoli into the pulmonary capillaries, where it can bind to hemoglobin for transport. Meanwhile, CO2 flows down its partial pressure gradient from the capillaries into the alveoli for expiration.

How would our respiratory systems adjust if we moved to higher altitudes where less oxygen is available? First, we would breathe more rapidly to try to avoid hypoxia; second, the binding dynamics of hemoglobin to oxygen would be altered to facilitate the unloading of oxygen at the tissues. As we will discuss in Chapter 7 of MCAT Biology Review, the natural response of hemoglobin to the decreased carbon dioxide concentration in the environment would actually be to decrease the unloading of oxygen to tissues, so other mechanisms must counteract and override this phenomenon. In the short term, the body can make more red blood cells to ensure the adequate delivery of oxygen. In the long term, the body could develop more blood vessels (vascularization), which would facilitate the distribution of oxygen to tissues.

Thermoregulation

In order to maximize gas exchange, there is a tremendous surface area over which the alveoli and capillaries interact. Because the entire respiratory tract is highly vascular, it can also be used for thermoregulation, or the regulation of body temperature. Heat—the transfer of thermal energy—is regulated via the body surfaces by vasodilation and vasoconstriction. As capillaries expand, more blood can pass through these vessels, and a larger amount of thermal energy can be dissipated. As capillaries contract, less blood can pass through them, conserving thermal energy. Nasal and tracheal capillaries are most frequently used for these purposes within the respiratory system. While these capillary beds provide a mechanism for thermoregulation, humans predominantly regulate temperature using capillaries and sweat glands in the skin, or rapid muscle contraction (shivering). The respiratory system can also transfer heat to the environment through evaporation of water in mucous secretions. Other animals, such as dogs, take advantage of this cooling mechanism by panting.

Immune Function

As mentioned above, the lungs provide a large interface for the body to interact with the outside world. While this is important for gas exchange and thermoregulation, it also comes with potential risks—pathogens such as bacteria, viruses, and fungi can cause infections in the lung, or can gain access to the body through the rich vascularity of the alveolar membranes. By necessity, the lungs must be able to fight off potential invaders. The first line of defense is the nasal cavity, which has small hairs (vibrissae) that help to trap particulate matter and potentially infectious particles. The nasal cavity also contains an enzyme called lysozyme. Also found in tears and saliva, lysozyme is able to attack the peptidoglycan walls of gram-positive bacteria. The internal airways are lined with mucus, which traps particulate matter and larger invaders. Underlying cilia then propel the mucus up the respiratory tract to the oral cavity, where it can be expelled or swallowed; this mechanism is called the mucociliary escalator.

MCAT EXPERTISE

The division within the sciences is largely artificial; the MCAT often contains questions that integrate multiple science disciplines. A question located in the Biological and Biochemical Foundations of Living Systems section may require knowledge of general chemistry. In fact, 10% of this section is chemistry: 5% general chemistry and 5% organic chemistry.

The lungs, especially the alveoli, also contain numerous immune cells, including macrophages. Macrophages can engulf and digest pathogens and signal to the rest of the immune system that there is an invader. Mucosal surfaces also contain IgA antibodies that help to protect against pathogens that contact the mucous membranes. Finally, mast cells also populate the lungs. These cells have preformed antibodies on their surfaces. When the right substance attaches to the antibody, the mast cell releases inflammatory chemicals into the surrounding area to promote an immune response. Unfortunately, these antibodies are often reactive to substances such as pollen and molds, so mast cells also provide the inflammatory chemicals that mediate allergic reactions.

REAL WORLD

Pneumonia is an infection of the lung most often caused by bacteria or viruses. Atypical pneumonia, commonly called walking pneumonia (because the infection does not require hospitalization and does not leave the patient bedridden), is often caused by a very small bacterium called Mycoplasma pneumoniae. This bacterium causes a prolonged cough because it damages epithelial cells lining the lung and paralyzes the cilia lining the respiratory tract. The lack of cilia makes it much more difficult to clear mucus from the lungs. The cough lasts until the respiratory epithelial cells have recovered and the cilia are once again functional.

Control of pH

The respiratory system plays a role in pH balance through the bicarbonate buffer system in the blood: CO 2  ( g ) + H 2 O  ( l ) ⇌  H 2 CO 3   ( a q ) ⇌  H +  ( a q ) + HCO 3 −  ( a q )

Questions regarding the bicarbonate buffer system are MCAT favorites, and you are very likely to see it in some form on Test Day. This equation represents an opportunity for the MCAT to test understanding of basic chemistry concepts, such as Le Châtelier’s principle, as well as how disturbances in pH may affect respiration.

The body attempts to maintain a pH between 7.35 and 7.45. When the pH is lower, and hydrogen ion concentration is higher (acidemia), acid-sensing chemoreceptors just outside the blood–brain barrier send signals to the brain to increase the respiratory rate. Further, an increasing hydrogen ion concentration will cause a shift in the bicarbonate buffer system, generating additional carbon dioxide. As described earlier, the respiratory centers in the brain are sensitive to this increasing partial pressure of carbon dioxide and will also promote an increase in respiratory rate.

REAL WORLD

Metabolic acidosis—a condition of excess acid by any mechanism besides hypoventilation—is a common occurrence in medicine. Anaerobic respiration can generate lactic acid; individuals with type 1 diabetes mellitus can produce ketoacids when they are hypoinsulinemic; certain poisons, like methanol and formaldehyde, can produce organic acids. In each of these cases, one of the primary methods of compensation is increasing respiration rate.

As the respiratory rate increases, more carbon dioxide is blown off. This will also push the buffer equation to the left, but notice the difference: the shift to the left in the previous paragraph was caused by an increase in hydrogen ion concentration, which elevated the concentration of carbon dioxide. Here, the removal of carbon dioxide causes a shift to the left that allows the hydrogen ion concentration to drop back to normal.

If the blood is too basic (alkalemia), then the body will seek to increase acidity. How can the lungs contribute to this? If the respiratory rate is slowed, then more carbon dioxide will be retained, shifting the buffer equation to the right and producing more hydrogen ions and bicarbonate ions. This results in a lower pH.

BRIDGE

If H+ is an acid and HCO 3 – is a base, then why doesn’t increasing both of them yield a constant pH? The reason is that H+ is a strong acid, while HCO 3 – is a weak base. Just like a titration, discussed in Chapter 10 of MCAT General Chemistry Review, this combination will shift the pH of the solution toward the acidic range.

Overall, the lungs play a role in the immediate adjustment of carbon dioxide levels and, by extension, hydrogen ion levels. However, the lungs do not work alone to maintain proper pH. The kidneys also play a role by modulating secretion and reabsorption of acid and base within the nephron. This is a much slower response, however, and represents long-term compensation. BIOLOGY GUIDED EXAMPLE WITH EXPERT THINKING

Chronic inhalation of crystalline silica has been shown to result in silicosis and pulmonary fibrosis and may also play a role in the development of pulmonary hypertension. The mechanism by which these pathologies develop is not clearly understood, but oxidant/antioxidant imbalances have been suggested. Extracellular superoxide dismutase (Sod3) is one of the most abundant antioxidant enzymes in the pulmonary vasculature. To study the role of Sod3 in the development of silica-mediated pathologies, scientists injected a crystalline silica suspension into the trachea of WT and Sod3–/– mice. 28 days later, a pressure  catheter was inserted into the right ventricle through the jugular vein to measure the right ventricular systolic pressure (RVSP) (Figure 1). Elevated RVSP is associated with pulmonary hypertension. Following the RVSP measurement, mice were sacrificed. mRNA was extracted from lung tissue and assayed for Sod3 expression (Figure 2). Lung sections were stained with Mason’s trichrome to visualize collagen, a marker of pulmonary fibrosis (Figure 3). Based on the findings of this study, does Sod3 protect against or exacerbate crystalline silica-induced pathologies?

This question is really testing our understanding of how the presence or absence of Sod3 affects the development of silica-induced lung pathologies. Finding an answer will require a strong understanding of the experimental design and careful analysis of the data. Let’s start with the experimental design. Paragraph 2 tells us that we have three experimental groups: normal mice not treated with silica, normal mice treated with silica, and Sod3 knockout (KO) mice treated with silica. The normal mice lacking treatment serve as a negative control, whereas the normal mice treated with silica serve as the positive control. Paragraph 1 tells us that crystalline silica has already been shown to cause pulmonary fibrosis and silicosis, so we should expect to see indicators of these pathologies in normal mice treated with silica, but not in the untreated mice. Because knockout of Sod3 lead to an increase in pulmonary fibrosis and pulmonary hypertension after exposure to crystalline silica, Sod3 most likely serves a protective role in the lungs against silica-mediated damage.

As we learn about the human body, it may be easy to reduce the complex and varied functions of the lungs to breathing and providing a supply of oxygen. The lungs do indeed perform gas exchange, which relies on differences in partial pressures of gases between the alveoli and the blood. Oxygen is taken up by the blood, while carbon dioxide is released for exhalation. Inhalation and exhalation also require pressure differentials created by anatomical structures such as the chest wall, diaphragm, pleurae, and lungs.

However, the lungs are so much more than just bags of air; gas exchange is not the only function of the respiratory system.
